• A statement under penalty of perjury that you believe the material was removed by mistake or misidentification.

• A statement that you consent to the jurisdiction of the federal district court where your address is located, or any jurisdiction where OnStream App may be found, and that you will accept service of process from the person who submitted the original DMCA claim.

OnStream App reserves the right to terminate accounts of users who are repeat copyright infringers, at our sole discretion.
